WSL Junior Cricket Registration Form

Please complete the following form to register your child(ren) for 2025 Junior Cricket. There will be 15 sessions this year, commencing on Monday 21st April (including bank holidays and half term).

Please note the following when answering the question about your child’s disabilities: the Equality Act 2010 defines a disabled person as anyone with ‘a physical or mental impairment, which has a substantial and long-term adverse effect on his or her ability to carry out normal day-to-day activities’.

By submitting this form, you are registering your child for the 2025 Junior Cricket and agreeing to the following:

  • I confirm I have legal responsibility for my child(ren) and I am entitled to give this consent. I confirm, to the best of my knowledge, all information provided on this form is accurate, and I will undertake to advise the club of any changes to this information.
  • I give my consent that in an emergency, the club may act in my place (loco parentis), if the need arises for the administration of emergency first aid (and/or) other medical treatment which, in the opinion of a qualified medical practitioner, may be necessary.
  • I understand that in such an occurrence all reasonable steps will be taken to contact me or the other adult (as detailed above).
  • I confirm to the best of my knowledge that the child(ren) in my care do(es) not suffer from any medical condition other than those detailed above.
  • I agree to pay £100 per child for the sessions. Payment options will be sent within 24 hours of receipt of the form to the parent email addresses provided.
